1) Your ACTUAL  passport (NO photocopy acceptable, as an E2 Visa must be stamped on the page of your passport)

2) Completed visa application form
‘who will pay for your trip?’  –> You write down that school will pay for the trip

3) Confirmation number of Visa issuance

4)  One recent passport type color photograph

5)  $45 USD (Cash or Money order acceptable)

6) Official Sealed transcript

7) Consul’s check list
